Funeral Planning Form

The family, relatives and friends of the deceased person should, if possible, plan the funeral service. Please choose the scripture readings from the selections found in Reading I, Reading II and Gospel Readings found on this website under Forms. Also, decide on who will read and who will  perform ritual actions listed on this form.Please indicate your choices below and submit at least one day prior to the funeral Mass.
